Your Crown Visit Planner
Arrive with your questions.
Leave with a clearer plan.
If you have been told a tooth may need a crown, you may be thinking about several things at once: keeping the tooth, feeling comfortable, finding time, and understanding the cost. You do not have to settle those questions before asking for an evaluation. Bring them into the conversation.
A crown covers a tooth to restore its shape and support it when it is weakened or damaged. The starting point is the tooth and the examination. A same-day crown can be convenient for a suitable case; your dentist can explain whether that approach fits your needs. ADA: Crowns, Cleveland Clinic: Dental Crowns.
This planner helps you prepare for that conversation at Allegra Dental Center. Print it or keep it open during your visit. You can leave any item blank.
Your concern
Tell the team what brought you in
Choose the details you want to discuss:
- A tooth feels different when I chew.
- A filling or tooth has chipped, broken, or changed.
- I have already received a crown recommendation and want to understand it.
- I want to discuss appearance, an existing crown, or a longer-term plan.
- Something else is worrying me.
Write down when you first noticed the change, what brings it on, and what you want the dentist to understand. Mention previous treatment and any advice you have already received. If the tooth or a crown has recently changed, contact the office about the timing of your visit rather than waiting to finish this planner.
Your priorities
Put your priorities in order
What would make the plan easier to understand or follow? Choose your top two:
- Protecting the tooth and understanding the recommendation.
- Feeling comfortable during the visit.
- Fitting appointments around work, caregiving, or travel.
- Understanding the appearance and material choices.
- Knowing the proposed cost and what is included.
- Understanding care after treatment.
Tell the team which matters most. A useful starting sentence is: “My biggest concern is ___, and I would like to understand ___ before deciding.”
The findings
Ask what the examination shows
Use these questions to connect the recommendation to your tooth:
- What did you find, and can you show me the area you are concerned about?
- What would a crown do for this tooth?
- Is a filling, another type of restoration, or monitoring a reasonable option here?
- What are the tradeoffs between those options for me?
- Is anything else needed before the final restoration can be planned?
The answer may involve a crown or a different next step. Ask the dentist to explain the reason in everyday language. To explore the different restoration types beforehand, read Inlay, Onlay, Crown, or Veneer?.
The visit
Plan the visit around your life
Same-day crowns are made with an in-office digital workflow in suitable cases. Some teeth benefit from a laboratory or staged approach. Ask which workflow is being recommended and why. Cleveland Clinic: Dental Crowns.
- Could the final crown be completed in one visit in my case?
- How much time should I set aside for the proposed appointment?
- If more than one visit is needed, what happens between visits?
- How can I tell you if I need a pause or feel uncomfortable?
- What should I know about eating, work, travel, and care afterward?
Share your calendar constraints before choosing a date. The office can explain the proposed visit arrangements. For a fuller comparison, read Same-Day Crown or Traditional Crown?.
The costs
Make the financial conversation concrete
Ask for the proposed plan and fee explanation before agreeing to treatment:
- What treatment and follow-up are included in the estimate?
- What additional work, if needed, would change it?
- Which insurance questions should be checked, and what remains uncertain?
- What payment arrangements apply to this plan?
- How do the costs and tradeoffs compare with the reasonable alternatives?
Bring your insurance information if you want the office to discuss benefits. Read the practice's Financial Policy and ask about terms that matter to your circumstances. An estimate and an insurance discussion answer different questions; ask the office to distinguish them clearly.
The next step
Leave with the next step written down
Before the visit ends, complete these four lines with the team:
- The recommended next step is: ____________________
- The reason for that recommendation is: ____________________
- My remaining question is: ____________________
- The timing and person to contact are: ____________________
Ask which changes or symptoms should prompt another call. Keep any personal notes with you; this planner does not need to be submitted online.
